The Dark Side of Health Sector – What started us?

India today is a booming economy and the third fastest growing in the world. At one level, India hopes to become a major force at the global stage, yet, over one third of income is spent towards food and related consumption and hence, social security support for health, education, housing etc. becomes critical. However, since independence, little has changed in context to changing the social security situation in the country. Though the government has taken numerous steps towards improving the living standards and the health situation of citizens in India, the impact has not been as profound as anticipated. The government of India has been making efforts through a galaxy of programs and initiatives over the years to improve the health situation of the nation. Though efforts are being made in the right direction, more needs to be done to achieve the desired goal set by the governments.

Nearly 75 per cent of dispensaries, 60 per cent of hospitals and 80 per cent of doctors are in urban areas, serving only 28 per cent of the Indian population, says a new report released by KPMG and the Organization of Pharmaceutical Producers of India (OPPI). The report also pointed out that non-communicable diseases (NCDs) account for nearly 60 per cent of deaths in India annually and the country is estimated to lose $4.58 trillion by 2030 due to them. And other report said that only 2% of doctors are in rural areas – where 68% of the population live.

No health research in India can be considered in isolation from the neglected state of health care for its citizens. India ranks 143 out of 184 countries for health-related Sustainable Development Goals. At 17% of the world’s population, India contributes disproportionally to the global burden of disease, with more deaths of children aged 5 years or younger than any other country (1·25 million), a quarter of the world’s tuberculosis, unhealthy lifestyles, premature cardiovascular disease, and an epidemic of diabetes. As if oblivious to the facts, government expenditure on health is one of the lowest in the world at 1·4% of gross domestic product. An amount wholly inadequate to train, staff, equip, or run health facilities.

Nutrition status has been dismal and is one of the causes of child mortality and morbidity. As per the global hunger index (by IFPRI), India ranks 78th among 118 developing countries (with 15% of our population being undernourished; about 15% under-5 children who are ‘wasted’ while the share of children who are `stunted’ is a staggering 39% and the under-5 mortality rate is 4.8% in India.)

While communicable diseases contribute 28% of the entire disease burden, non-communicable diseases (60%) show ample rise and injuries at (12%) now constitute the bulk of the country’s disease burden.

India ironically has to cater two extreme healthcare situations. First one is arising out of exclusions (out of poverty or lack of proper healthcare facilities) and second one is lifestyle diseases like diabetes and cardiac related problems.

India’s ratio of 0.7 doctors and 1.5 nurses per 1,000 people is dramatically lower than the WHO average of 2.5 doctors and nurses per 1,000 people. The majority of health workforce is concentrated in urban areas. Furthermore, there is an acute shortage of paramedical and administrative professionals too.
